This September, the YMCA of the Triangle is partnering with Triad Care to offer convenient Flu Shot Clinics at select YMCA branches. These regional immunization clinics are open to YMCA staff and members.
In addition to flu shots, COVID-19 vaccines will also be available at these clinics, while supplies last. Vaccine availability may vary based on CDC recommendations and distribution guidelines, which are subject to change.
To ensure availability and reduce wait times, we encourage you to reserve an appointment by clicking on your preferred clinic location.

Completed Immunization Form
Please print and fill out this form prior to your appointment. If multiple vaccines are offered at your clinic, indicate which vaccine(s) you wish to receive.
Insurance Information
Bring a copy of your insurance card or the physical card. If you are a covered spouse or dependent, include the primary plan member’s name and date of birth on your form.
Clothing
Wear short sleeves or loose-fitting clothing to allow easy access for vaccination.
Hydration & Medication
Drink plenty of water and take your regular medications as usual on the day of the clinic.
Age-Specific Flu Vaccine Request
If you are 65 years or older, please indicate on your form if you would like to receive the high-dose flu vaccine.
